Standards of Practice

Standard
To the greatest extent possible, Continuum Senior Care Management, Inc. (Continuum) will work to foster self-determination on the part of the older person, to enable the person to live in accordance with his or her personal values and goals.  

Client
While the primary client usually is the older person whose care needs have instigated the referral to a professional geriatric care manager, all others affected by his/her care needs should be considered part of the "client system."

The primary client may not necessarily be the person who makes the initial contact or the person responsible for payment for services rendered.
 
Members of the client system may include:
  • the older person
  • a family member within or outside of the older person's household
  • a paid caregiver
  • friends, neighbors or community agencies
  • a third party with fiduciary responsibilities
  • other professionals, such as a physician, a nurse from a home health care agency, an attorney, etc.

Conflicts
In the event of conflicting needs within the client system, the goal of professional intervention should be to strive for resolution through a process of review and discussion among the parties, facilitated by the National Association of Professional Geriatric Care Managers (PGCM).

Provision of service
In working with each client (Client), Continuum Senior Care Management, Inc. (Continuum) will provide ongoing service to each client only after Continnum has assessed Client's needs and Client, or a person designated to act for Client, understand and agree to a plan of service, the results that may be expected from it, and the cost of service.

Self-determination
Continnum will base its plan of service on goals Client, or a person designated to act for Client, has defined, and which enhance the decisions Client has made concerning Client's life.

Loyalty
The first duty of Continnum is loyalty to Client. Continuum will always provide services based on Client's best interest, even if this conflicts with the interests of Continuum or others.

Termination of service
Continuum will end service to Client only after reasonable notice. Continuum will recommend a plan for Client to continue to receive services as needed.

Substitute judgment
Continuum will not substitute its judgment for Client's unless Continuum is acting in the role of Client's guardian, appointed by a court of law, or with Client's approval, or the approval of someone designated to act for Client.

Confidentiality
Continuum will hold in trust any confidence Client provides to continuum, disclosing information to others only with Client's permission, or if Continuum is compelled to do so by a belief that Client will be seriously harmed by Continuum's silence, or if the laws of this state require Continuum to do so.

Referrals/disclosure
Continuum will refer Client only to services and organizations Continuum believes to be appropriate and of good quality. Continuum will fully explain to Client any business relationship Continuum has with any service Continuum proposes, and give Client information on alternatives, if at all possible, so that Client, or a person designated to act for Client, can make an informed decision to accept or reject the services Continuum recommends to Client.

Cooperation
Continuum will strive to ensure cooperation between all of the individuals involved in providing service and care to Client.

Qualifications
Continuum is fully qualified in its profession to provide the services it undertakes. Care managers of Continuum shall continue to improve their skills and knowledge by participating in professional development programs and maintaining certification and licensing in their profession.

Discrimination
Continuum will not promote or sanction any form of discrimination.
