An elderly person's health, mental well-being, and ability to thrive are often best accomplished by helping them retain their existing home life and supporting them in such a way as to enhance their happiness while respecting their autonomy. Our goal is to see that your loved ones receive the right care and support — from household chores to help with medication to daily companionship. Our unique care management approach is designed so that the right support is there to meet our clients' ever-changing needs.


Assessment
We begin with a standardized assessment, which includes an in-person visit by one of our Care Managers, covering a range of issues including health, security, emotional, financial situations, and legal requirements.

Tailored Care Plan
Continuum's Care Managers create a care plan specific to each elderly person's needs and culture. We review the plan with the person being cared for and family members.

We coordinate and manage appropriate services. We spot issues before they become problems. And we provide regular reassessments and revisions to the care plans. We see to it that your elder loved ones constantly receive the best care and support possible.

Services available
Sometimes, what's needed is simple, such as help with groceries, housework, grooming, or transportation. In other cases, additional care and support is needed dealing with issues such as safety, companionship, medications, bill paying, or legal assistance. In some cases comprehensive care is needed, for instance, for those with conditions such as physical handicaps, Alzheimer's or other dementia, or failing health.

We provide hands-on services including but not limited to:
